4. Complete and submit supporting application documents

• Copy of applicant's birth certificate
• Recent photograph of applicant
• Hawaii Health Form 14 [Download form]
• Student-Athlete Medical Clearance [For ARA Excell applicants only]
• Copy of previous school year’s complete report card and any recent standardized test results, if available.
• If applying to Grades 10 – 12, Official transcript of all high school courses.
